Dignity. Connection. Belonging.

Where people are seen
and systems work.

Bread & CommUnion is a human-centered Systems of Care practice that integrates direct support with relational community and systems change — existing at the intersection of provision and belonging.

You care deeply about the people you serve. Let's make sure your systems reflect that care.

Where systems and lived experience meet.

For organizations ready to bring care closer to the people they serve.

Your mission is rooted in care—but do people feel it?

We work alongside your team and community to find what's getting in the way, and help build support people can access, trust, and shape.

THE WORK

A bridge between good intentions
and people’s everyday lives.

When services are difficult to navigate or people feel unheard, the gap is relational as well as practical. We listen before we design — and co-create support people can access, trust, and help shape.

PARTNERSHIP OPPORTUNITIES

Ways we can work together

01 / LISTEN

Understand where people get stuck

Listening with community members and staff to identify barriers, confusing processes, and moments when people feel unheard.

  • Community listening
  • Systems assessment
  • Service navigation review
  • Barrier identification
02 / CO-CREATE

Shape a program with the people it serves

Bringing lived experience into the design or improvement of a program, service, or community gathering.

  • Program design
  • Community engagement
  • Community experience design
  • Community outreach
03 / PUT INTO PRACTICE

Bring care into everyday practice

Supporting teams through training, implementation, and ongoing partnership so dignity and relationship shape daily interactions.

  • Team training
  • Program implementation
  • Ongoing partnership and support

Each partnership begins with listening to your context and shaping the work together.

Let’s talk about what you’re noticing

WHERE PEOPLE AND SYSTEMS MEET

The Bridge

We translate lived experience into policy and institutional design—connecting the community’s table with the systems that shape care.

FROM SYSTEMS OF CARE TO ECOSYSTEMS OF CARE

Care grows through relationship.

An ecosystem of care holds the formal and the informal: institutions, neighbors, families, and the relationships that sustain everyday life. It is shaped by a place, its history, and its people. Care moves in many directions.

An ecosystem of care unfolding from pencil sketch into watercolor on an artist’s desk, with an older hand drawing an inclusive community gathering.
Provision meets belonging.

NEAR HORIZON

Trans-forming systems now

Bring the community’s table into institutional decisions. The Bridge carries lived experience into the work of reducing harm, building trust, and centering dignity today.

NEAR-FUTURE HORIZON

Ecosystems of care, be-coming

Nurture interdependent ways of caring that are already taking shape. The Table expands beyond institutions into the relationships among communities, places, and the land—with people shaping what comes next.

We are also in relationship with land, water, history, what came before us, and what we leave behind. An ecosystem of care makes room for all of it.

HOW WE APPROACH OUR WORK

With C.A.R.E.™

How care is delivered is inseparable from whether it works.

C.A.R.E.™ is our framework for systems transformation. Each practice names what a system builds and makes possible. Beneath it lives a deeper posture—the soul layer that keeps the practice honest and the system accountable.

Care begins with a posture of the heart, moves into how people are treated, shapes relationships, and arrives at what we build together.

Dignity Affirmer
Community Alchemist
Advocate of Our Shared Humanity

“I couldn’t unsee the gap between what was intended and what was actually experienced.”

Rooted in care
Committed to our shared humanity.

I work where community
and systems meet.

My commitment to this work comes from proximity — a lifelong Journey that put people before paperwork, a petition that once reached the Governor's office and ended with a barrier permanently removed from a state program. Not a policy win for its own sake — one less hurdle between people and the access they're owed. I carry that same commitment as a community advocate — standing beside people to pave a dignified path forward.

As the daughter of immigrants, I grew up interpreting between worlds — translating school notices, sitting between two languages at parent-teacher conferences, making the phone calls my parents couldn't. I know how much it matters to be met with clarity, compassion, and respect, because I know what it costs when you're not. Those experiences shape my commitment to building support that people can trust and truly belong within.

I founded Bread & CommUnion to hold those things together — immediate support, relational community, and systems change, in the same breath. Bread meets immediate needs. Communion builds the belonging that sustains.

This work keeps growing through community encounters and lived experience. It has structure. But the people and places it meets are what keep it alive.
